Starting Hourly Pay Range: $19.00 - $20.27

Work Hours: Monday through Friday, 8:00 a.m. - 5:00 p.m.

The Care Coordinator is part of a multidisciplinary clinical care team and responsible for collaboration with clinical team members, patients, and families to facilitate unmet health care needs within the context of the Patient Centered Medical Home (PCMH) model at NATIVE HEALTH. The Care Coordinator answers all calls from patients with clinical questions, providing excellent customer service and resolving patient inquiries in a timely manner.

KEY ACCOUNTABILITIES:

• Answers Health Services patient inquiries from multiple entry points and resolves the patient’s needs on a timely basis.  
• Provides Care Coordination. 
• Promotes Preventive Care and Chronic Disease Follow Up. 
• Promotes the Patient Centered Medical Home  
• Supports the organization as needed.  

QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:  

• Associate degree in a health-related field is required.. 
• 2 years of experience in the medical field is required. 
• Ability to read, comprehend and write instructions, correspondence, and memos. 
• Fluency in both English/Spanish preferred. 
• Ability to gather data in an organized fashion from varied sources. 
• Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment. 
• Must have an updated Immunization (IZ) Record, including COVID-19 vaccines.
