Job Description

The Integrated Behavioral Health Consultant works under the supervision of the Medical Director and performs all functions, duties, and responsibilities within the primary care department as a member of the care team. The consultant provides quality clinical mental health consultation to providers and patients as requested by the providers.

RESPONSIBILITIES AND DUTIES:

  1. Provides clinical consultation to primary care providers (PCP) and staff for adults, children, adolescents, and families, regarding mental and behavioral health related issues that are directly connected to the patients’ medical condition(s). 
  2. Works with PCP to develop a treatment plan that addresses the patients’ behaviors and mind-body connection as these relate to their medical condition(s).
  3. Conducts brief (15-30 minute), behavioral health interventions tailored to the patients’ medical problems, focusing on Active Development of Behavioral Change planning and self-management during the patient visit to the PCP.
  4. Makes appropriate referrals for patients to services such as diabetes education, case management, Health Start, and Home Visiting.
  5. Collaborates with that staff as needed to assist in addressing patients’ needs.
  6. Provides follow up appointments separately and/or in conjunction with medical provider visits related to the Behavioral Change Plan. 
  7. Works with medical provider to identify patients’ behavioral health conditions that impact patients’ overall health, and makes internal and external referrals as appropriate to behavioral health services such as psychiatry, counseling, and addiction services. 
  8. Responsible for crisis triage and intervention.
  9. Conducts warm hand-offs to mental/behavioral health and addiction services throughout the organization. 
  10. Tracks the completion of internal and external patient referrals to behavioral health and addiction services.
  11. Collaborates and co-manages the patient’s behavioral health care with consistent, expeditious feedback to referring providers. 
  12. Documents the clinical behavioral health aspect of patient interventions and completes notes in a timely manner for billing purposes.

ADDITIONAL RESPONSIBILITIES

  1. Participates in staff development activities by attending workshops, seminars and conferences, and other activities as assigned in order to improve patient care skills.
  2. Assures that patients receive services in an appropriate, efficient, expeditious and professional manner.  Recognizes the need for development and modification of services and proposes/recommends changes to supervisor.

Qualifications

QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:

  1. Current, valid and unrestricted license of independent practice in the State of Arizona – LCSW/LPC is required.
  2. A master’s degree in social work or related discipline from an accredited program is required.
  3. Three years of experience as a behavioral health provider is required.
  4. Experience working with a diverse population in a community health setting is preferred.
  5. Must have a valid and unrestricted Arizona Driver’s License (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid AZ driver license during employment. Must be insurable by NATIVE HEALTH liability auto policy.
  6. Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment.
  7. Must provide a negative Tuberculosis (TB) skin test upon employment and provide a TB skin test annually.
  8. Must provide an immunization card.
  9. Must obtain a CPR and First Aid Card within ninety (90) calendar days of employment and maintain valid cards during employment.

KNOWLEDGE, SKILLS AND ABILITIES:  The following factors are essential to this position, and in order to minimally qualify, applicants must provide evidence in their resumes that they possess these required factors:

  1. Knowledge of and the ability to provide integrated service delivery to patients involving social and emotional problems utilizing motivational interviewing.
  2. Knowledge of the cultural, social, and economic backgrounds of therapeutic and crisis intervention services involving social and emotional problems stemming from medical conditions or diagnosis.
  3. Ability to provide written or oral reports and evaluations concerning program activities, accomplishments, goals and objectives.
  4. Knowledge of existing social and behavioral health resources available for patient care.
  5. Capable of making independent professional judgments and recommendations for medical, social and psychosocial intervention.
