Job Description

The Home Visitor with the Home Visiting Program will provide developmental information, support and encouragement to individuals and families with a primary focus on pregnant women, infants, young children and expectant fathers; perform case management services as directed utilizing the Parents As Teachers home visitation model; case management services will include client intake, data collection and input, facilitation and development of service plans including functions for advocacy, crisis management, referrals and directing families to local resources for health care, substance abuse, mental health, housing, food, job training programs and insurance providers. This position works under the administrative supervision of the Home Visiting Program Lead.

Starting Salary: $17.49

KEY ACCOUNTABILITIES:

  1. Provides the coordination and delivery of services to enable health and well-adjusted childhood development.
  2. Identify and enroll Native American families with children under the age of five into the program.
  3. Assists in the planning and facilitation of group activities and meetings that focus on child development, parent-child interaction, and education on family well-being.
  4. Documents and keeps records of all visits and case management services provided to families.
  5. Supports the organization as needed.

Qualifications

QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:

  1. Must possess a Bachelor’s degree in early childhood development/education, social work, public health, or related discipline appropriate to this position.
  2. Must possess two years of specialized experience working with young children and/or parents, preferably within the American Indian/Alaskan Native community.
  3. Must possess a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver’s license during employment.
  4. Must be insurable under NATIVE HEALTH’s liability auto policy.
  5. Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment.
  6. Must pass a substance abuse test upon employment and submit to random testing during the course of employment.
  7. Must have an updated Immunization (IZ) Record.
  8. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
  9. Must obtain a First Aid/CPR Card within (90) calendar days of employment and maintain a valid CPR Card during employment.

  1. Knowledge of the social, cultural, and economic backgrounds of the American Indian/Alaskan Native population.
  2. Knowledge of client confidentially and client rights according to Heath Insurance Portability and Accountability Act (HIPAA) and Family Education Rights and Privacy Acts (FERPA).
  3. Excellent oral and written communication skills.
  4. Excellent interpersonal skills.
  5. Trained in Parents as Teachers curriculum a plus.
  6. Must be flexible and adaptable in meeting the needs of low-income families.
  7. Ability to make independent and professional judgment or recommendation for medical, social and behavioral intervention.
  8. Ability to effectively present information and respond to questions/concerns from parents.
  9. Ability to effectively communicate developmental issues to parents.
