Since then the agency has expanded to include Primary Care Division (Medical/Dental) and Behavioral Health Division.

NATIVE HEALTH currently provides a wide range of programs including primary medical, dental, WIC (available at three sites), health promotion and prevention, wellness programs and behavioral health programs.

We provide services to over 17,000 individuals per year, resulting in over 70,000 visits. NATIVE HEALTH is a private, non-profit 501(c)(3) organization and is led by a Chief Executive Officer under the supervision of the Board of Directors.

Job Description

The Licensed Practical Nurse (LPN) - Nursing Supervisor is a licensed professional who is responsible for assessing patient needs; provides direct and indirect nursing care. The position works closely with the Practice Manager in monitoring the day to day operation of the medical clinic ensuring effective and efficient flow of the back office activities. The LPN-Nursing Supervisor supervises the Medical Assistants (MA) and Referral Coordinator and works under the primary supervision of the Practice Manager.

KEY ACCOUNTABILITIES:

  1. Monitors day-to-day clinical operations of the medical clinic.
  2. Responsible for the immunization program at clinic site:
  3. Provides nursing services to patients in accordance with the Arizona State Board of Nursing LPN scope of practice.
  4. Functions as NHW Employee Health Nurse
  5. Supports the Organization as needed.

Qualifications

QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:

  1. Graduate of an accredited school or college of nursing is required.
  2. Licensed to practice nursing in the state of Arizona as LPN is required.
  3. At least three (3) years nursing experience is required. Two or more years in a supervisory position is preferred.
  4. Experience in a primary care setting is preferred.
  5. Bilingual in Spanish is preferred.
  6. Advanced computer skills including knowledge of software applications such as Outlook, MS Word, EXCEL, PowerPoint, ACCESS, and ability to operate computers
  7. Experience in conducting a performance appraisals for employee under supervision
  8. Must be dependable, punctual, relate well with others and able to meet deadlines
  9. Must be flexible in work hours to include some Saturday hours
  10. Must be 25 years to drive and be insurable by NATIVE HEALTH liability auto policy.
  11. Must have a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years.  No more than two at-fault accidents in the last three years.  Must maintain a valid AZ driver license during employment.  Must be insurable by NATIVE HEALTH liability auto policy.
  12. Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) days of employment.
  13. Must pass a drug and alcohol testing upon employment and random testing during the course of employment.
  14. Must have updated Immunization (IZ) record. 
  15. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an update annually.
  16. Must obtain a CPR Certificate within ninety (90) days of employment and maintain a valid card during employment.
