The Native American Community Health Center, Inc. (dba NATIVE HEALTH) was started in 1978 as a small community nursing program. The initial funding was a grant from Indian Health Service. Since then the agency has expanded to include Primary Care Division (Medical/Dental) and Behavioral Health Division.

NATIVE HEALTH currently provides a wide range of programs including primary medical, dental, WIC (available at three sites), health promotion and prevention, wellness programs and behavioral health programs.

We provide services to over 17,000 individuals per year, resulting in over 70,000 visits. NATIVE HEALTH is a private, non-profit 501(c)(3) organization and is led by a Chief Executive Officer under the supervision of the Board of Directors.

Job Description

The Outreach and Enrollment Specialist (OES) is responsible for assisting families and children of the community to identify, enroll, and access affordable health insurance options through Medicaid/CHIP and the Health Insurance Marketplace.  Most duties will be conducted in an outreach oriented environment in efforts to diminish rates of uninsured children.  The OES works in conjunction with the Lead Outreach Enrollment Specialist to recruit new schools, community partners and sites in outreach, enrollment and retention activities and the development of new strategies and approaches to improve outreach and enrollment of eligible patients.  Approximately 75% of work activities will require travel and participation in evening and/or weekend activities in facilitating outreach to American Indian and Alaska Native families throughout Maricopa County with an emphasis on the East Valley.  The OES will represent the company professionally and positively to enhance and promote the core values and mission of the organization, always exercising utmost discretion, diplomacy and tact in patient/staff interactions. 

KEY ACCOUNTABILITIES:

  1. Participates in advocacy and outreach and enrollment activities that promote services available through NATIVE HEALTH with a focus on understanding and gaining access to affordable health insurance options through Medicaid/CHIP and the Health Insurance Marketplace.
  2. Provide marketing flyers, announcements and educational materials to the Community at large within Maricopa County and ensure that materials are culturally competent, clear, and useful prior to dissemination through school or outreach events.
  3. Monitor enrollment status of individuals and families to ensure completion of application process and maintain data to ensure families renew eligibility annually.
  4. Collaborate with other local, regional, and/or state wide outreach enrollment assistance efforts.
  5. Assist clients with enrollment and/or referrals to appropriate public health programs.  This may require in-home, in-clinic or community follow up.
  6. Supports the organization as needed.

Qualifications

KNOWLEDGE, SKILLS AND ABILITIES:

  1. Knowledge of eligibility requirements of AHCCCS (Medicaid) and CHIP (KidsCare) and other alternate medical and social service programs/benefits available.
  2. Ability to uphold quality customer service to all patients/customers of NATIVE HEALTH.
  3. Experience with community outreach and/or education.
  4. Must be highly flexible to accommodate the changing needs of the department.
  5. Demonstrate proficiency with computer based applications such as MS Office, Adobe, spreadsheets, email and other computer software programs.
  6. Demonstrate the ability to manage multiple schedules, timelines and deadlines with flexibility and minimal supervision.
  7. Ability to learn eligibility, enrollment, and access barriers to medical care issues of the Native American communities of Maricopa County.
  8. Ability to work with multiple levels of supervision and differing program requirements simultaneously.
  9. Ability to communicate with co-workers and superiors in order to carry out assignments, meet goals and insure the success of the Outreach and Enrollment Program.

QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:

  1. Associate Degree required in a related field.  Bachelor’s degree preferred.
  2. Bilingual in English/Spanish languages a plus.
  3. Capacity for professional level of customer service.
  4. Proficiency in use of general office equipment.
  5. A minimum of one year of combined experience in patient registration, advocacy, and outreach and education.
  6. Must have experience using PC based, networked computer software and hardware and other office automation tools.
  7. Must have good communication skills, both written and verbal.
  8. Capacity for self-motivation and ambition with minimal supervision (work in the field and outreach).
  9. Must understand the Native American community and culture, as well as experience working with people from diverse ethnic groups and various backgrounds.
  10. Must be 25 years of age and have a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years.  No more than two at-fault accidents in the last three years.  Must maintain a valid AZ driver license during employment.  Must be able to drive agency vehicle to outreach locations.  Must be insurable by NATIVE HEALTH liability auto policy.
  11. Must have the ability to work evenings and weekends.
  12. Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment.
  13. Must pass a substance abuse testing upon employment, and submit to at random testing during the course of employment.
  14. Must have an updated Immunization (IZ) Record.
  15. Must submit a recent report of Tuberculosis (TB) skin test upon employment and provide a TB skin test annually.
  16. Must obtain a CPR card within ninety (90) calendar days of employment and maintain a valid CPR card during employment.
