The Native American Community Health Center, Inc. (dba NATIVE HEALTH) was started in 1978 as a small community nursing program. The initial funding was a grant from Indian Health Service. Since then the agency has expanded to include Primary Care Division (Medical/Dental) and Behavioral Health Division.

NATIVE HEALTH currently provides a wide range of programs including primary medical, dental, WIC (available at three sites), health promotion and prevention, wellness programs and behavioral health programs.

We provide services to over 15,000 individuals per year, resulting in over 68,000 visits. NATIVE HEALTH is a private, non-profit 501(c)(3) organization and is led by a Chief Executive Officer under the supervision of the Board of Directors.

Job Description

POSITON SUMMARY: The Circle of Care Program Manager is responsible for planning, implementing, and managing the Circle of Care program at Native Health under the Community Health and Wellness Division and in collaboration with all Native Health Departments and programs as appropriate. The Program Manager facilitates program planning and design from implementation through evaluation for the program as outlined within the grant funded deliverables.  The Program Manager reports directly to the Community Health and Wellness Director.

KEY ACCOUNTABILITIES:

  • Responsible for planning, program design, and ensuring alignment with Native Health's mission and strategic plan.

Measures of Success:

  1. Program activities and services are designed to support the delivery of expected outcomes as defined by official Notices of Grant/Program Award.
  2. Programs/grants objectives, activities, deliverables, and reporting are completed in a timely manner and meet expectations as defined by the funder. 
  3. Maintains consistent and timely communication with the funder’s project officers/grants management representatives, direct supervisor, and Native Health Senior Management as appropriate.
  4. Leads program development and grant proposal development activities in collaboration with Native Health’s development staff, community representatives, and Native American youth and families.

  1. Acts as a lead partner in developing the activities into the NATIVE HEALTH infrastructure and Urban Native American Communities.

Measure of Success:

  1. The established Community Steering Committee is representative of targeted populations and is actively engaged and working with Circle of Care staff towards the development of a culturally responsive evidenced based practice model.
  2. Circle of Care staff is committed to integration efforts within the culture of Native Health.
  3. Appropriate training is developed and delivered to staff, community members, steering committee, and Native Health departments to facilitate an integrated model that ensures quality culturally responsive care.

  1. Leads and manages staff through mentoring, coaching, and supervision ensuring staff engagement to meet organizational and program goals and objectives.

Measures of Success:

  1. Staff demonstrate alignment with organizational mission and values.
  2. Staff meet productivity benchmarks as outlined within individual job descriptions and program deliverables.
  3. Staff are properly oriented and receive appropriate training to ensure successful performance. Staff is qualified and meet required competencies to fulfill positions.
  4. Staff receive timely and constructive feedback that will enhance performance.
  5. Leads team building, staff engagement, and plans strategies to facilitate integration of services into the Native Health and targeted community culture.
  6. Program is fully staffed.  Hiring, orientation, and training meet established policies and procedures.

  1. Oversees program compliance, budgets, and program performance

Measures of Success:

  1. Assures quality and compliance with policies and procedures and with funder’s rules, regulations, and standards.
  2. Completes required monthly, quarterly, semi-annual, and annual reporting mandates for the program, meeting established requirements.
  3. Interprets and trends statistical data and provides recommendations for improvement and/or sustainability. Notifies department director of any changes, discrepancies, and or concerns related to program adjustments, additions, and/or modifications.
  4. Manages the program’s grant funded budget.  Program remains fiscally responsible for grant/program revenue and expenses. Financial reporting is accurate and completed on a timely basis.

  1. Collaborates with external agencies and organizations building strong partnerships.

Measures of Success:

  1. Acts as the community liaison for NATIVE HEALTH Circle of Care program in Maricopa County.
  2. Develop and implement a consistent and ongoing program promotion/educational materials to be disseminated to community stakeholders, Native Health providers, Urban Native American communities, and other potential community resources.
  3. Provides internal and community culturally competent presentations and trainings.
  4. Oversees the development and implementation of a community advisory committee consisting of community stakeholders and youth and families who may be involved in services.
  • Supports the organization as needed.

KNOWLEDGE, SKILLS, AND ABILITIES:

1.     Advanced knowledge of community-based system of care philosophy, practice, and responsiveness.

2.     Proficient knowledge of program development concepts and project management methodologies.

3.     Knowledge of the cultural and social background and the social determinants of health of the Urban Native American population.

4.     Ability to relate effectively with people at various levels including government departments, schools, community organizations, corporations, the general community, and youth and families.   

5.     Demonstrated ability to communicate effectively, orally and in writing.

6.     Ability to organize, plan and work independently.

7.     Ability to supervise, mentor and coach staff, volunteers, and interns. 

8.     Ability to establish rapport and gain the confidence of Urban Native American youth and families, community stakeholders, and culturally diverse advocates.

9.     Must have capacity to network, develop meaningful collaborations and coordinate activities across multiple programs and agencies.

10. Must be able to work with confidential material without betraying the trust of providers, participants, families, or the agency. 

11. Capacity to comprehend complex concepts and think globally.

12. Creativity and ability for abstract thinking.

13. Ability to work with minimal supervision while meeting all requirements of the position.

14. Ability to establish program goals and objectives and to assess progress toward their achievement.

15. Proficient use of software applications such as MS Word, EXCEL, PowerPoint, and Outlook.

16. Experience working with EHR systems is a plus.

17. Must be dependable, punctual, relate well with others and able to meet deadlines.

Qualifications

QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:

1.     A minimum of a Bachelor’s degree in public health, behavioral/mental health, social work or other related health discipline is required; Master’s level preferred.

2.     Educational and experiential knowledge of Substance Abuse/Mental Health Prevention or Substance Abuse/Mental Health Treatment Services for youth/young adults/families is required. 

3.     Three to five years of directly related experience is preferred. A minimum of two years of experience in supervision, mentoring, and professional development of employees is required. 

4.     Possess a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years.  Must maintain a valid AZ driver license during employment. Must be insurable by NATIVE HEALTH liability auto policy.

5.     Must have completed a criminal background check and obtained a Class I Fingerprint Clearance Card at the time of employment start date.

6.     Must have a current (within 12 months) Tuberculosis (TB) skin test report upon employment and provide an update annually. 

7.     Must provide an updated immunization (IZ) record at the time of employment. 

  1. Must obtain a CPR Card within ninety (90) calendar days of employment and maintain a valid CPR card during employment.
  2. Must be willing to submit to random drug testing during employment.
