Job Description
We are looking for a passionate and motivated Provider who supports the Mountain Family mission to start in December or January of this year. The position is full time—4 days/week, 10 hours per day—in our clinic in Glenwood Springs. Additionally, our status as a CHC in an underserved area meets qualifications for both state and federal loan repayment programs. The clinic is well supported by an excellent nursing staff and bilingual MAs. The ideal candidate for this job is someone who is interested in serving the under served and has a track record for working well with all employees across all departments.
Qualifications
A. Education or Formal Training & Experience:
a. Masters degree in Nursing if FNP
b. Certification by the American Nurses’ Association as a Family or Adult Nurse Practitioner if FNP
c. Physician Assistant degree if applying as a PA
d. Current Colorado medical license required.
e. Minimum Three (3) years in a family practice setting preferred.
f. ACLS Certification preferred; BLS required.
B. Knowledge, Skill & Ability:
a. Working knowledge of equipment typically used at the Health Centers.
b. Ability to work independently in professional manner.
c. Excellent organizational, verbal and written communication and customer service skills.
d. Ability to understand and present verbal instructions and to exchange verbal information is essential.
e. Ability to work in a team environment.
f. Ability to establish and maintain effective, courteous working relationships with patients, staff team members, insurance entities and others.
g. Ability to work under pressure in a fast-paced environment
C. Physical Requirements and Workplace Environment:
a. Employment contingent on being free of active tuberculosis. This position requires yearly tuberculosis testing.
b. Requires lifting, pushing, pulling and carrying up to 35 pounds.
c. Essential to have ability to hear routine conversations.
d. Requires movement from room to room.
e. Requires periods of standing and walking while seeing patients.
f. Requires sufficient near vision to be able to read documents and computer screen.
g. Essential to have ability to stoop, kneel, bend, crouch, twist and reach.
h. Essential to have ability to comprehend both oral and written communications.
i. This position has an O.S.H.A. classification of Category 1: All procedures or other job-related tasks involve an inherent potential for mucous membrane or skin contact with blood, body fluids or tissues or a potential for spills or splashes of these fluids.
j. Must pass a criminal background check
k. Annual Flu vaccine required. Employer will provide
l. Obtain 2 step PPD for tuberculosis (annually if provider). Employer will provide

This is a 1.0 FTE position
Annual salary hiring range is $87,000 - $96,000 depending on experience.
