Job Description

We are seeking a compassionate and skilled Wellness Nurse (LPN) for a part-time day position in our healthcare facility located in Southbury, United States. As a vital member of our healthcare team, you will provide holistic nursing care to our residents, focusing on their overall well-being and quality of life.

  • Monitor the health, safety, and well-being of all residents
  • Provide direct nursing care with an emphasis on holistic wellness
  • Assist in training and monitoring medication administration
  • Supervise care staff in accordance with current state regulations and community policies
  • Maintain clinical quality assurance in line with federal, state, and local standards
  • Conduct thorough resident assessments
  • Collaborate with the Health and Wellness Director (RN) to ensure optimal care delivery
  • Promote the highest degree of service to our residents while demonstrating the mission of the company
  • Assist in the supervision of day-to-day nursing activities performed by Certified Nursing Assistants
  • Ensure proper documentation of all nursing interventions and resident care

Qualifications

  • Graduation from an accredited School of Nursing with a current LPN license
  • Minimum of 2 years' experience in long-term care, assisted living, home health, or hospital setting preferred
  • Experience working with residents with Alzheimer's or other related dementias
  • Strong knowledge of federal and state regulations, and nursing practices for health and wellness
  • Proficiency in computer skills, including Microsoft Office (Windows, Outlook, Excel)
  • Excellent communication and interpersonal skills
  • Ability to handle multiple tasks and work efficiently in a fast-paced environment
  • Demonstrated ability to work independently and as part of a collaborative team
  • Current CPR certification
  • Physical requirements: ability to stand, walk, sit, use fine and gross motor skills, reach, balance, stoop, kneel, crouch, and lift or carry weight up to 50 lbs
  • Flexibility to adapt to changing resident needs and work schedules
  • Empathetic and patient-centered approach to care
  • Detail-oriented with a strong commitment to accuracy in medication administration and documentation
