Job Description
We are seeking a compassionate and skilled Wellness Nurse (LPN) for a part-time day position in our healthcare facility located in Southbury, United States. As a vital member of our healthcare team, you will provide holistic nursing care to our residents, focusing on their overall well-being and quality of life.
- Monitor the health, safety, and well-being of all residents
- Provide direct nursing care with an emphasis on holistic wellness
- Assist in training and monitoring medication administration
- Supervise care staff in accordance with current state regulations and community policies
- Maintain clinical quality assurance in line with federal, state, and local standards
- Conduct thorough resident assessments
- Collaborate with the Health and Wellness Director (RN) to ensure optimal care delivery
- Promote the highest degree of service to our residents while demonstrating the mission of the company
- Assist in the supervision of day-to-day nursing activities performed by Certified Nursing Assistants
- Ensure proper documentation of all nursing interventions and resident care
Qualifications
- Graduation from an accredited School of Nursing with a current LPN license
- Minimum of 2 years' experience in long-term care, assisted living, home health, or hospital setting preferred
- Experience working with residents with Alzheimer's or other related dementias
- Strong knowledge of federal and state regulations, and nursing practices for health and wellness
- Proficiency in computer skills, including Microsoft Office (Windows, Outlook, Excel)
- Excellent communication and interpersonal skills
- Ability to handle multiple tasks and work efficiently in a fast-paced environment
- Demonstrated ability to work independently and as part of a collaborative team
- Current CPR certification
- Physical requirements: ability to stand, walk, sit, use fine and gross motor skills, reach, balance, stoop, kneel, crouch, and lift or carry weight up to 50 lbs
- Flexibility to adapt to changing resident needs and work schedules
- Empathetic and patient-centered approach to care
- Detail-oriented with a strong commitment to accuracy in medication administration and documentation
