Job Description

PRINCIPAL ACCOUNTABILITIES:

  • Provide comprehensive clinical and community-based support to individuals with mental health diagnoses.
  • Conduct ongoing assessments of clients' needs and develop individualized service plans, including diagnostic reviews, every 180 days.
  • Offer crisis intervention and support to clients in need.
  • Assist clients in accessing essential resources, including benefits, housing, and community support services.
  • Provide education and support to clients and their families regarding symptom management and medication adherence.
  • Maintain accurate and timely documentation of all services provided, including progress notes and billing.
  • Complete required assessments, including crisis plans, DLA-20, GAIN, and Reaching Recovery assessments.
  • Participate in an on-call rotation to provide after-hours support to clients.
  • Advocate for clients within social, governmental, and community systems.
  • Collaborate with a multidisciplinary team and participate in staff meetings and training sessions.
  • Maintain client confidentiality and adhere to ethical standards.
  • Achieve and maintain productivity standards, with opportunities for monthly bonuses.

Qualifications

QUALIFICATIONS

  • Master's degree in Social Work, Counseling, Psychology, or a related field required.
  • Experience in a mental health or social services setting preferred.
  • Strong desire to pursue licensure (LGPC, LGSW, etc.).
  • Ability to work flexible hours, including evenings and weekends, as needed.

SPECIALIZED KNOWLEDGE AND SKILLS REQUIREMENTS:

  • Knowledge of mental health diagnoses and treatment modalities.
  • Strong clinical assessment and intervention skills.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively with diverse populations.  
  • Proficiency in electronic health records (EHR) and Microsoft Office Suite.  
  • Commitment to ethical practice and client advocacy.

  • Competitive starting salary of $43,000 - $50,500 annually, based on experience.
