Medical Case Manager (Bachelor's Level Position)
The McClendon Center, founded in 1980, is a certified community based service provider operating under the District of Columbia Department of Behavioral Health. As a leading not-for-profit organization in Washington, DC, the McClendon Center provides outpatient behavioral rehabilitation to the mentally ill by fostering creativity, promoting friendship, instilling independence, and providing an outlet for emotional growth and greater participation in the community. Accredited by Joint Commission on Accreditation of Healthcare Organizations (JCAHO), the McClendon Center operates multiple programs including Day Program Services and the Core Services Agency (CSA).

McClendon Center conducts outreach on behalf of District Medicaid Managed Care Organizations (MCO). This outreach is targeted toward MCO members who have been frequent users of emergency rooms, typically for non-emergency reasons. Members who are successfully outreached receive a telehealth behavioral health evaluation by a licensed clinician. Those who have apparent behavioral healthcare needs are referred to McClendon Center’s Core Services Agency. Those who have only medical needs require short-term case management in order to connect with their primary care physician (PCP) and specialty care. The Medical Concierge outreaches these members to facilitate follow-up appointments with their providers and accompanies them to their first visit following the emergency room visit that triggered outreach.
1. Once a member has been referred for Medical Concierge service, the Medical Concierge contacts her/him on the same day.
2. Ensure that the member has consented to care coordination services (for medical purposes), not mental health services.
3. Determine where the member has been referred for post-ER follow-up, as well as who her/his PCP is.
4. Assist the member in making appointments for post-ER follow-up and for PCP visit.
5. Ensure that any prescriptions issued in the emergency room are filled.
6. Refer the member to "My Health GPS".
7. Set up transportation for medical visits. When it is apparent that the member may not utilize the transportation service, accompany the member to the appointment.
8. Offer Mom’s Meals for members with a stable address.
9. Continue to follow the member until he/she can confirm that the PCP appointment has been kept, and when My Health GPS enrollment has been confirmed.
10. Provide extended support (beyond the iniitial PCP visit) only when requested by AmeriHealth Caritas DC.
11. Document all activities in Credible (EMR) the same day the service was rendered.
• Bachelor’s Degree in a social science or related field.
• One (1) to four (4) years of experience in a social service or nonprofit environment. Experience in a mental health or similar agency is highly desired.
• Ability to work flexible hours as required, including evenings and weekends.
• Valid Driver's License, Active Auto Insurance Coverage & a Working Vehicle.
SPECIALIZED KNOWLEDGE AND SKILLS REQUIREMENTS:
• Ability to coordinate with a multi-disciplinary team within an outpatient mental health agency.
• A superior communicator, who shares information readily and concisely, listens as well as gives advice, and respects the abilities of others.
• Knowledge of sound ethical practices and principles related to the care of mental health consumers.
• Excellent written and verbal communication skills.
• Excellent computer skills, to include operating Microsoft Office Suite programs.
• Knowledge and ability to navigate an Electronic Medical Record (EMR), including data entry.
• Ability to relate to adults diagnosed with serious and persistent mental illness, using knowledge of concepts, principles, and theories related to mental health and mental illness.
• Ability to perform amicably in an environment serving the seriously and persistently mentally ill adult population.
• Ability to respect and uphold client confidentiality.
• Ability to exhibit a high level of organizational skills and able to prioritize and multi-task without direct supervision.

Physical Demands: The position requires moderate physical effort on a daily basis such as walking, bending, stooping and standing for periods of time.
