Job Description

The Community Health Nurse works collaboratively with physicians, staff, and other health care professionals to provide care coordination across the healthcare continuum for all consumers. As an integral member of the team, the Registered Nurse works to ensure that safety, best practices, and high-quality standards of care are maintained across the continuum. Responsible for the seamless delivery of care in a manner consistent with the organization’s mission and within the scope of practice of licensure held. Is responsible for completing client assessments at intake and ongoing including coordinating a wide range of self-management support and disease management activities for the clients throughout the Core Service Agency, Professional services group, Day Program, Patient Discharge Coordination team as is medically necessary to ensure quality care delivery. This candidate will provide services where and when they enhance client care and stability. This includes but is not limited to the office, day program, hospitals, group homes, home setting and other locations as needed. The nurse collaborates with pharmaceutical companies, pharmacies, clinics, and other similar entities for physical and mental health care coordination.


  • Ability to coordinate with a multi-disciplinary team within an outpatient mental health agency.
  • Experience in program design and implementation in a nonprofit or clinical setting.
  • Knowledge of sound ethical clinical practice and principles related to the care of mental health consumers.
  • A superior communicator, who shares information readily and concisely, listens as well as gives advice and respects the abilities of others.
  • Excellent written and verbal communication skills.
  • Excellent computer skills, to include operating Microsoft Office Suite programs.
  • Knowledge and ability to navigate an Electronic Medical Record (EMR), including data entry.
  • Ability to relate to adults diagnosed with serious and persistent mental illness, using knowledge of concepts, principles, and theories related to mental health and mental illness.
  • Ability to perform amicably in an environment serving the seriously and persistently mentally ill adult population.
  • Ability to respect and uphold client confidentiality.
  • Ability to exhibit a high level of organizational skills and ability to prioritize and multi-task without direct supervision.
  • Ability to navigate the District of Columbia by privately owned vehicle, public transportation or other suitable means that allow the timely and efficient delivery of services.


  • Licensed as a Registered Nurse (RN) or APRN in the District of Columbia.
  • A minimum of two (2) years of experience in nursing, specifically medical-surgical nursing, clinical and operational leadership. 
  • Must have strong clinical knowledge, data analysis, conflict resolution, and excellent writing skills.
  • A minimum of one year of community Health Nursing preferred
  • One year of Mental Health experience preferred.
