Job Description

The Care Coordinator provides support to the Health Home team and individual consumers as part of the implementation of care plan activities. The Care Coordinator functions as a member of an interdisciplinary team to provide care coordination to a caseload of severely mentally ill adults with co-morbid medical disorders.


•Provide and assist in the provision of Health Home services as described in the care plan.

•Coordinate behavioral health care, substance abuse, and health care services informed by evidence-based clinical practice guidelines, including prevention of mental illness and substance use disorders.

•Coordinate access to preventive and health promotion services.

•Coordinate access to chronic illness management, including self-management support to individuals and their families.

•Advocates for and supports the client, engages with community agencies/health care providers and others on his or her behalf to ensure access to services needed to increase wellness self-management and reduce emergency room visits and/or hospitalizations.

•Provides clinical support to the Team by providing consultation, education, information around psychosocial and/or substance abuse conditions, interventions and resources to maintain focus on outcomes and best practices.

•Provides interventions for consumers, including but not limited to: skills development; crisis intervention; substance abuse assessment, education and referral; financial management; household management and preventative maintenance; preventive health; preventive mental health; educational/vocational assessment and planning; resource generation.

•Develops linkages with other support systems.

•Works as part of a Care Coordination team, attends and participates in team meetings to provide input/feedback around psychosocial conditions to review client status, updates plans and goals, and reviews outcomes to further program goals.



• Ability to coordinate with a multi-disciplinary team within an outpatient mental health agency.

• Knowledge of sound ethical clinical practice and principles related to the care of mental health consumers.

• A superior communicator, who shares information readily and concisely, listens as well as gives advice, and respects the abilities of others.

• Excellent written and verbal communication skills.

• Excellent computer skills, to include operating Microsoft Office Suite programs.

• Knowledge and ability to navigate an Electronic Medical Record (EMR), including data entry.

• Ability to relate to adults diagnosed with serious and persistent mental illness, using knowledge of concepts, principles, and theories related to mental health and mental illness.

• Ability to perform amicably in an environment serving the seriously and persistently mentally ill adult population.

• Ability to respect and uphold client confidentiality.

• Ability to exhibit a high level of organizational skills and able to prioritize and multi-task without direct supervision.


• Bachelor’s degree in health or public health-related field.

• One (1) year of full-time behavioral health experience or a combination of education and relevant experience.

Additional Information

Starting Salary: $37K - $41K annually, based on experience

Physical Demands: The position requires moderate physical effort on a daily basis such as walking, bending, stooping and standing for periods of time.
