The McClendon Center, founded in 1980, is a certified community based service provider operating under the District of Columbia Department of Behavioral Health. As a leading not-for-profit organization in Washington, DC, the McClendon Center provides outpatient behavioral rehabilitation to the mentally ill by fostering creativity, promoting friendship, instilling independence, and providing an outlet for emotional growth and greater participation in the community. Accredited by Joint Commission on Accreditation of Healthcare Organizations (JCAHO), the McClendon Center operates two programs: Day Program Services and the Core Services Agency (CSA).
McClendon Center operates programs in two locations, a day program near Metro Center and an outpatient service clinic in the NOMA section of DC. The Center has revenues of approximately $4.5 million, with contracts from the Department of Behavioral Health, three Managed Medicaid Organizations, and grants from various foundations.
McClendon Center is a respected and highly rated agency in the District of Columbia, having received the highest rating on the Department of Behavioral Health’s provider scorecard three out of the last five years. The Center is also the only independent mental health agency in the District to be accredited by the Joint Commission on Accreditation of Healthcare Organizations.

Must Possess a Valid Driver's License & Insured Vehicle
• Ensure the proper notification of the client’s hospitalization to all required parties and inform care partners at AmeriHealth DC.
• Travel to locations in the community within required time frame to engage with clients post-admission.
• Obtain signed release forms and other required documentation from clients and coordinate appropriate follow-up care with the client’s outpatient providers.
• Enter any diagnostic information obtained from community providers into the client’s medical record, attaching any required notes and supporting documentation regarding the diagnosis.
• Liaison with clients’ outpatient providers, communicating all relevant information relating to the admission and discharge to the client. Provide consumers with informed service options in the community.
• Act as primary support system during consumer’s transition into the community.
• Provide consumers with education around medication management and problem solve any conflicts with filling medications in a timely manner.
• Secure the earliest possible appointment for the client to be seen by an outpatient psychiatrist.
• Collaborate with clients’ outpatient provider(s) to meet any needs or concerns identified by the client.
• Meet with Clinical Manager for regular supervision, as required, and update Clinical Manager on client’s status post-discharge from an inpatient unit.
• Schedule interval visits to clients’ home or place of residence, as assigned, following the discharge date until the post discharge requirements have been met.
• Make referrals to the Center’s Day Program for clients who need daily structure and rehabilitation following discharge.
• May perform other duties as assigned.
Specialized Knowledge, Skills & Requirements
• Coordinate behavioral health care, substance abuse, and health care services informed by evidence-based clinical practice guidelines, including prevention of mental illness and substance use disorders.
• Coordinate access to preventive and health promotion services.
• Coordinate access to chronic illness management, including self-management support to individuals and their families.
• Advocates for and supports the client, engages with community agencies/health care providers and others on his or her behalf to ensure access to services needed to increase wellness self-management and reduce emergency room visits and/or hospitalizations.
• Provides interventions for consumers, including but not limited to: skills development; crisis intervention; substance abuse assessment, education and referral; financial management; household management
and preventative maintenance; preventive health; preventive mental health; educational/vocational assessment and planning; resource generation.
• Develops linkages with other support systems.
• Bachelor’s degree required.
• Two (2) to Four (4) years of relevant experience.
• Experience with community integration preferred.
• Experience working in a psychiatric setting highly desired.
• Possession of a valid drivers' license and working vehicle at all times, with minimum insurance coverage as required by state law.

Physical Demands: The position requires moderate physical effort on a daily basis such as walking, bending, stooping and standing for periods of time.
