Job Description

The Nurse Care Manager works collaboratively with physicians, staff and other health care professionals to provide health home and care coordination across the health care continuum for all consumers. An integral member of the health home team, the Nurse Care Manager works to ensure safety, best practice and high quality standards of care are maintained across the continuum. Responsible for coordinating a wide range of self-management support and disease registry activities for the health home's entire consumer population. Success will be measured by the results of the process and outcome performance measures of the population of consumers in the clinic.


• Managing the coordination of care for clients through the supervision of Care Coordinators.
• Demonstrates ability to recognize and assess emergency needs of consumers and initiate appropriate emergency procedures as needed.
• Acts to facilitate interdisciplinary communication. Utilizes other members of the team in assessing the consumer and analyzes consumer data. Assesses learning needs of the consumer and significant others. Gives accurate and complete information to consumer and family.
• Evaluates consumer and family understanding of treatment plan and/or instructions.
• Provides individual care management and health promotion as well as population data management activities.
• Maintains appropriate technique for preparation, procedures and medications administered.
• Accurately and legibly documents all consumer interactions in consumer record according to center procedure.
• Reports results and pertinent information to consumer and team members.
• Applies professional nursing skills in the provision of preventive health maintenance and treatment of ill and injured consumers
• Conducts comprehensive assessment of consumer's physical health history
• Participates in staff organizational meetings.
• Provides direct clinical services to clients on an individual, group, and family basis in the office and in community settings.
• Provides on-call services for Health Home consumers and serves as backup to evening and weekend staff.
• Attends staff meetings, workshops, and seminars to learn agency policy, rules, regulations, and procedures; participates in on-going in-service training as well as required external training.
• Maintains core competencies in relation to working with co-occurring disorders through continuing education and implementing skills into all aspects of treatment.
• Establish and maintain positive, effective working relationships with other senior management, outside agencies, and the medical and mental health community.
• May perform other duties as directed.


Salary: Negotiable, based on relevant experience

•Ability to coordinate with a multi-disciplinary team within an outpatient mental health agency.
•Experience in program design and implementation in a nonprofit or clinical setting.
•Knowledge of sound ethical clinical practice and principles related to the care of mental health consumers.
•A superior communicator, who shares information readily and concisely, listens as well as gives advice, and respects the abilities of others.
•Excellent written and verbal communication skills.
•Excellent computer skills, to include operating Microsoft Office Suite programs.
•Knowledge and ability to navigate an Electronic Medical Record (EMR), including data entry.
•Ability to relate to adults diagnosed with serious and persistent mental illness, using knowledge of concepts, principles, and theories related to mental health and mental illness.
•Ability to perform amicably in an environment serving the seriously and persistently mentally ill adult population.
•Ability to respect and uphold client confidentiality.
Ability to exhibit a high level of organizational skills and able to prioritize and multi-task without direct supervision.

Must be licensed as a Registered Nurse (RN) or APRN in the District of Columbia.
•A minimum of two (2) years of experience in nursing, clinical, and operational leadership, and solidly demonstrated management skills. 
•Must have clinical management knowledge, data analysis, conflict resolution, and excellent writing skills.
•Resident of the District of Columbia, preferred.

Additional Information

Physical Demands: The position requires moderate physical effort on a daily basis such as walking, bending, stooping and standing for periods of time.
