Job Description

Must Possess a Valid Driver's License & Personal Vehicle

The Care Coordinator provides support to the Care Management Team and individual AmeriHealth  Caritas DC members as part of the implementation of care plan activities. The Care Coordinator/CM functions as a member of an interdisciplinary care management team to provide care coordination and extended care management to a caseload of members with co-morbid medical and mental health concerns.

PRINCIPAL ACCOUNTABILITIES:

  • Provides and assists in the provision of care coordination and care management services as described in the plan of care
  • Coordinates behavioral healthcare, substance abuse, health care, and social services as needed by members.
  • Coordinates access to  preventive and health promotion services.
  • Coordinates access to chronic illness management, including self-management support to individuals and their families.
  • Advocates for and supports the client, engages with community agencies/health care providers and others on his or her behalf to ensure access to services needed to increase wellness self-management and reduce emergency room visits and/or hospitalizations.
  • Provides clinical and logistical support to the Care Management Team as directed by the Care
    Manager Lead.
  • Provides interventions for consumers, including but not limited to: skills development; crisis
    intervention; substance abuse assessment, education and referral; financial management; household management; preventive health; preventive
    mental health; educational/vocational assessment and planning; resource generation.
  • Develops linkages with other support systems.
  • Provides a maximum of 25% of services in the office, and a minimum of services face-to-face and/or in the community.
  • Documents all activities in AmeriHealth’s electronic health record, JIVA. All services are documented within four  hours of having been delivered.
  • Works 9am to 5pm three days per week, and 11am to 7pm two days per week, Monday through Friday. Every third week, works 9am to 5pm on Saturday.

Qualifications

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Bachelor’s degree in health or public health-related field.

  • Valid Driver's License & a Fully Insured Personal Vehicle.

  • One (1) year of full-time behavioral health experience or a combination of education and relevant experience.

  • Resident of the District of Columbia, preferred.

SPECIALIZED KNOWLEDGE AND SKILLS REQUIREMENTS:

  • Ability to coordinate with a multi-disciplinary team within an outpatient mental health agency.

  • Knowledge of sound ethical clinical practice and principles related to the care of AmeriHealth members.

  • A superior communicator, who shares information readily and concisely, listens as well as gives advice, and respects the abilities of others.

  • Excellent written and verbal communication skills.

  • Excellent computer skills, to include operating Microsoft Office Suite programs.

  • Knowledge and ability to navigate an Electronic Medical Record (EMR), including data entry.

  • Ability to respect and uphold client confidentiality.

  • Ability to exhibit a high level of organizational skills and able to prioritize and multi-task without direct supervision.
