As an RN Clinical Case Manager at MGA Homecare, you’ll combine your clinical expertise and leadership skills to oversee and guide patient care. You’ll empower your team, serve as a trusted resource for families, and play a pivotal role in improving patient outcomes.

Responsibilities Include:

• Leadership & Supervision: Guide and mentor a team of RNs, LPNs, and other home care professionals to deliver exceptional care.
• Training & Development: Provide clinical education to staff, ensuring best practices and compliance with state guidelines.
• Patient Advocacy: Coordinate care plans and ensure seamless communication between families, caregivers, and healthcare providers.
• Support & Guidance: Educate and empower families to promote patients’ physical and emotional well-being.
• Compliance & Safety: Uphold the highest standards of care and report concerns as required by law.

What You’ll Bring

• Active RN License in the state where you’re applying
• Home Health Experience required
• Minimum of 2 years of nursing experience
• Current BLS CPR certification
