Job Description
As an RN Clinical Case Manager at MGA Homecare, you’ll combine your clinical expertise and leadership skills to oversee and guide patient care. You’ll empower your team, serve as a trusted resource for families, and play a pivotal role in improving patient outcomes.
Responsibilities Include:
- Leadership & Supervision: Guide and mentor a team of RNs, LPNs, and other home care professionals to deliver exceptional care.
- Training & Development: Provide clinical education to staff, ensuring best practices and compliance with state guidelines.
- Patient Advocacy: Coordinate care plans and ensure seamless communication between families, caregivers, and healthcare providers.
- Support & Guidance: Educate and empower families to promote patients’ physical and emotional well-being.
- Compliance & Safety: Uphold the highest standards of care and report concerns as required by law.
Qualifications
What You’ll Bring
- Active RN License in the state where you’re applying
- Home Health Experience required
- Minimum of 2 years of nursing experience
- Current BLS CPR certification
