Based in the heart of downtown Lowell, Massachusetts Lowell Community Health Center is currently seeking a Full-Time Nurse Community Health Worker/Care Coordinator.

Job Description

This position combines administrative abilities and Substance Use Disorder (SUD) recovery coaching responsibilities in an outpatient setting. This position helps promote and maintain a well-coordinated and efficient operation in an atmosphere that promotes a positive and professional experience for our patient population. The OBAT SUD Recovery Coach performs a variety of clinical support functions to facilitate the smooth flow of operations. As an integral member of the patient care team, has a special role in working closely with the providers in their relationship with the patient and facilitates contact and support with the patient following referral to the OBAT Program. The Recovery Coach works with the OBAT clinical care team to ensure that the goals of the Health Center mission are accomplished. The Recovery Coach, he/she will provide recovery support services which will assist patients in achieving sustained recovery. This position will facilitate patient access to recovery-oriented SUD treatment, resources and community recovery groups. The recovery Coach will bridge treatment systems into a more holistic care model through providing support across multiple systems of care. The recovery support services designed to generate individualized recovery options and assist patients to remove challenges to/of recovery.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

· Provide peer support and mentoring in the recovery process

· Provides and helps ensure the provision of excellent customer service to both external customers (e.g., patients, hospital staff, etc.) and internal customers (providers, medical records staff, business staff, etc.)

· Distributes appropriate medical forms to clients for documentation. Assists patients, when needed, in completing State required forms and other forms as needed. Requests other staff to assist patient when language appropriate.

· Attends OBAT staff meetings and Lowell CHC all-staff meetings. Attend appropriate training programs as available.

· Performs other job-related duties as assigned, including, but not limited to other administrative responsibilities as assigned.

· Function, if necessary:

a. Welcomes patient at each appointment, takes clients vital signs and urine sample for drug screen in a professional and courteous manner.

b. Interviews patients to obtain information concerning medical history or other pertinent information.

c. Assists OBAT Nurse as necessary in the following areas: perform laboratory tests, such as pregnancy tests a and maintains laboratory logs per LCHC standards; take and record height and weight blood pressure, pulse, respiration and O2 saturation.

d. Assist with Vivitrol coordination and assure that Vivitrol medication is coordinated through the appropriate pharmacy on-site prior to client appointments.

e. Documenting all information in accordance with established requirements.

f. Maintains infection control practice at all times, especially with patients requiring isolation/reverse precautions. Maintains universal precautions in handling blood/body fluids. Maintains TB precautions according to clinic policy.

· Excellent organizational skills (oral and written) and the ability to excel at details and multi-tasking and the knowledge, skill and/or ability required to successfully complete the essential duties and responsibilities.
