Job Description

LifeTies, Inc. is hiring for a full time Residential Support Professional- Medical Case Management (RSP-MCMT)Trainee to work in residential home in Ewing (Mercer County) with adolescent youth ages 16-21 who may identify as LGBTQ+, have chronic health conditions, and/or have behavioral health needs.

Hours:  Sunday through Thursday 7am-3pm

Pay rate:  $17.00/hr

Responsibilities and Duties

The RSP-MCMT is charged with the duty to provide quality health care and medical case management, as well as genuine care and concern; evidenced based behavior support; and teach life-skills for our persons served who range in age from 16-21.  

Essential Functions: Include the following, other duties may be assigned

  • Participating in treatment meetings, providing medical reports and updates
  • Developing relationships with doctors, nurses and other medical health providers in service to youth.
  • Tracking data on youth health and entering data into electronic records system
  • Auditing medical files and medication storage
  • Supervising daily interactions in the group home and in the community.
  • Fostering positive relationships with persons served, families, team members, providers and other community members.
  • Serving as advocates for persons served; and teaching them to appropriately self-advocate.
  • Facilitating life skills activities (cooking, cleaning, budgeting, shopping, organization).
  • Completing well written electronic documentation of activities in the home, behaviors of persons served, and  life skills activities.
  • Administering first aid and CPR as needed.
  • Ensuring the privacy and dignity of persons served.

 

Qualifications

High School Diploma or GED with 3 years of experience; or BSW/Bachelors in behavioral science with 1 year experience; or a Medical Assistant w/ 2 years experience; or LPN with 1 year experience: working in an organization or school setting with youth ages 16-21 with behavioral health challenges; LGBTQ+; and/or with chronic health conditions.  Must have a driver's license in good standing; complete criminal and other background checks.

Sign On Bonus

$1000.00

The bonus will be paid in two installments, half will be paid on the next regularly scheduled pay date after you start employment with LifeTies, Inc., the other half will be paid 12 months  after date of hire.   The sign on bonus is taxable, and all regular payroll taxes will be withheld.  In the event you leave LifeTies, Inc. within 12 months of your date of hire, you will be responsible for reimbursing LifeTies, Inc. for the entire sign on bonus.
