Job Description

The Sr. Case Manager (CM) must possess a Bachelor's degree in behavioral health (social work/psychology, sociology, etc.); have three years of experience working with high functioning youth and young adults; and experience working with the populations served at LifeTies, Inc.  Interest in and experience with LGBTQ+ youth is highly preferred. 
Bilingual (Spanish-speaking) candidates are highly preferred.

The CM must have a valid driver’s license; pass several background checks; and be fully vaccinated and/or boosted for COVID 19.

Qualifications

MAJOR RESPONSIBILITIES:

The CM’s primary responsibility is to ensure that youth have a positive an enriching treatment experience at Triad by overseeing the following:

- Education, Training and Employment

- LGBTQ Advocacy and commitment to serving on community based committees

- Recreational activities and asked therapies

- Treatment planning/meetings and Family Engagement activities

- Social events, youth holidays, vacations, and community based leaves and activities

- Data entry; surveys and assessments

- Youth council, house meetings 

- Contacting stakeholders in the interest of youth

SUPERVISED BY: Program Coordinator

SUPERVISES:  Bachelors level Interns;  and direct support or Case Management trainees

JOB RESPONSIBILITIES and DUTIES INCLUDE BUT ARE NOT LIMITED TO:

 

  • Supervising Bachelors level interns, volunteers, and assistants
  • Complying with regulatory standards and contractual agreements
  • Providing some supervision for youth and transportation for some activities
  • Providing a minimum of 1 hour of case management services per youth per week; 1 life skills group per week
  • Preparation and dissemination of assessments, updates, reports, and treatment plans at designated intervals. 
  • Facilitating referrals to community agencies for supplemental services
  • Coordinating and facilitating educational groups and workshops (Psychoeducational); and allied therapies (art, music physical, etc). 
  • Utilizing an electronic records database or all record keeping  
  • Community networking  
  • Participating in scheduled agency staff meetings and agency/community workshops and training
  • Supervision of  persons served
  • Facilitating mediation and behavioral support plans for persons served
  • Support direct care staff to implement case management plans
  • Serving as a memeber of the group home administrative team

Employee Classification:  Full-time (40 hours a week).

Salary/Compensation:  $47,000 annually plus $1000 Sign On Bonus:

The bonus will be paid in two installments, $500 will be paid on the next regularly scheduled pay date after you start employment with LifeTies, Inc., $500 will be paid 12 months after date of hire.   The sign on bonus is taxable, and all regular payroll taxes will be withheld.  In the event you leave LifeTies, Inc. within 12 months of your date of hire, you will be responsible for reimbursing LifeTies for the entire sign on bonus.
