Housing Case Manager is a full-time position.

Job Description

  • Conduct screening of youth experiencing homelessness within 24-hours of referral.
  • Assist consumers as advocates for benefits, housing needs
  • Assist consumers in the development of positive working relationships with other agencies serving those at risk of homelessness.
  • Assist with identifying rental properties and developing positive working relationships with property owners.
  • Translate property owner tenancy expectations to consumers.
  • Provide life skills to consumers.
  • Effectively manage a caseload.
  • Develop and monitor Individual Service Plans with each consumer to meet their basic needs.
  • Maintain complete and accurate records on all consumers.
  • Familiar with and manage case data by entering consumer data into the Homeless Management Information System (HMIS) and agency databases in a timely manner.
  • Work in collaboration with team members to discuss best practices for more effective service delivery to consumers.
  • Attend agency staff meetings, TH/RRH meetings, trainings and other meetings as specified.
  • Performs other duties as assigned.

 

     

      Qualifications

      Qualifications

      • Minimum of a Bachelor’s degree in Social Work or related field.
      • 2 years of professional experience working with homeless youth and young adults accompanied/unaccompanied) 18 years and older population in case management capacity.
      • Knowledge of local community resources.
      • Reliable transportation and valid driver’s license.
      • Excellent verbal and written communication skills.
      • Must have flexible schedule to work occasional evenings and weekends.
      •  Self-motivated, reliable, strong attention to detail, maintain confidentiality and complies with all agency policies and procedures.

        Salary and Benefits

        Salary: $40,000-$42,000 annually

          Benefits for full-time employees include:
