ABOUT LIFE PITTSBURGH –

LIFE Pittsburgh, a growing nonprofit organization with approximately 400 employees, is a community-based alternative to nursing home care and assisted living. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community. Our health teams of geriatric physicians and nurse practitioners, nurses, social workers, rehabilitation specialists and other health care professionals assess our Participants' needs and desires, plan and approve services with them and their family or caregiver, monitor for changes and provide timely interventions. Primary care and community services are provided through our Day Health Centers and through our in-home program according to an individual Participant's needs.

Hourly Rate – CNA - $13.50 - $17.00

Non CNA - $12.50 - $15.38

RESPONSIBLE FOR –

• Providing care to meet the needs of the frail elderly to promote optimal health and maximize independence and self-care by providing or assisting participants with basic nursing and personal care services including, but not limited to bathing, grooming, toileting, feeding and assistance with any other ADL's.
• Care services for Participants are provided at their place of residence, and/or the Day Health Center.

SHIFTS –

• Immediate availability for 1 pm -9:30 pm shifts.
• Part Time Shifts Available – Daylight, Evenings, Weekends.
• Required to work every other weekend (Would receive a day off each week)

LOCATION/TRAVEL REQUIRED –

• Positions based out of an Adult Day Health Center (located in North Shore, Allegheny Center, McKees Rocks and Green Tree)
• Travel to multiple Participants residences throughout a shift is required. Service areas vary.
• All business mileage is reimbursed at the Federal rate.

REQUIREMENTS –

• High School Diploma or GED
• Valid PA Driver’s License AND Access to a Vehicle
• Current Auto Insurance
• Minimum 1 year experience providing direct care within the Elderly/Frail Population in a healthcare facility.
