Job Description

The mission and focus of the KVH Community Based Care is on providing care appropriate for the senior patient in their home setting - focusing on comfort and helping to support patients in achieving their life goals at a time when they are challenged by medical and mobility problems associated with aging. 

The APC will provide assessment and management of acute and chronic health problems for residents of skilled nursing facilities, assisted living facilities, and for home-bound patients. A high level of communication and collaboration with primary care physicians, patients’ families, facilities’ staffs, community agencies and KVH staff are fundamental aspects of this position.

Qualifications

REQUIRED QUALIFICATIONS

  • Diploma or degree from an accredited school of nursing.
  • Current RN and ARNP/FNP/AGNP/PA-C licensure in Washington State.
  • Prescriptive authority commensurate with licensure.
  • Valid Washington State driver's license.
  • Evidence of auto insurance in amounts compatible with KVH policy.

PREFERRED QUALIFICATIONS

  • Completion of accredited mid-level practitioner program, with a specialty in geriatrics.
  • Experience working with geriatric patients who reside in a skilled nursing facility, and assisted living facility or who are homebound.

Additional Information

Wage: Wage range is currently between $111,963 - $160,000 a year with Individual/Team Productivity Incentives, Individual/Team Patient Satisfaction Incentives and Retention bonuses. Actual wage will be based on years of applicable experience. 

FTE:  1.0 = 40 hours/week
