The Continuous Care LPN provides 1:1 care of Hospice patients in their home to help manage patient symptoms following the plan of care established by the Hospice RN and the patient's physician.  Following a training period the Continuous Care LPN would be placed in a "pool" and be called as needed.

What is required...

• Current LPN license from the Washington State Department of Health
• Current CPR card (maintenance required)
• One year nursing experience or three years of Medical Assistant experience
• Previous exposure to an Electronic Medical Records system
• Valid Washington State Driver’s license and reliable transportation

What is preferred...

• Previous home health or hospice experience

Wage Range:  Wage starts at $20.18/hour + 15% in lieu of benefits; actual wage will depend on years of applicable experience

FTE: Per Diem; variable shifts
