Job Description

The Geriatric Nurse Practitioner (GNP) provides assessment and management of acute and chronic health problems for residents of skilled nursing facilities, assisted living facilities, and for home-bound patients. A high level of communication and collaboration with primary care physicians, patients’ families, facilities’ staffs, community agencies and KVH staff are fundamental aspects of this position.

Qualifications

REQUIRED QUALIFICATIONS

  • Diploma or degree from an accredited school of nursing.
  • Current RN and ARNP licensure in Washington State.
  • Prescriptive authority commensurate with licensure.
  • Valid Washington State driver's license.
  • Evidence of auto insurance in amounts compatible with KVH policy.

PREFERRED QUALIFICATIONS

  • Completion of accredited mid-level practitioner program, with a specialty in geriatrics.
  • Experience working with geriatric patients who reside in a skilled nursing facility, and assisted living facility or who are homebound.

JOB KNOWLEDGE, SKILLS & ABILITIES

  • Ability to establish and maintain effective working relationships with the parties named in the Position Summary.
  • Excellent communication skills in English, both verbal and written.
  • Evidence of integrity, sound medical judgment, initiative, flexibility to respond to changing situation and unanticipated patient care needs.
  • Ability to communicate effectively with elderly adults who have visual, hearing, speech, and tactile impairments.

DUTIES & RESPONSIBILITIES

  • Visit patients at facility sites for routine examinations, intakes, and updates.
  • Diagnose and treat problems within the scope of practice and expertise. Assess nursing/functional needs of patients in addition to medical diagnosis.
  • Respond to voice mail routinely Monday through Friday for SNF and ALF concerns, AM and
    PM.
  • Communicate with SNFs and ALFs staff for routine concerns in communication book weekly.
  • Respond to family questions and concerns related to patient care in coordination withthe physician.
  • Record visits and turn in billing logs weekly.
  • Promote discharge planning for patients, when appropriate.
  • Manage medications in collaboration with PCP
  • Perform mental status examinations, if patients are able to participate.
  • Promote GNP Project both within facilities and the community.
