Job Description

Manage Social Care Clinic Services, providing care for the community and collaborating with social care providers. Managing the accreditation process and other quality and risk management activities.

Client Assessments

Assess the needs, strengths, and challenges of clients

Gather relevant background information through interviews, and consultations.

Assess employees’ psychosocial needs through interviews, surveys, and consultations.

Develop tailored intervention plans to address individual or team challenges

Support and Counselling

Offer emotional support and counselling to individuals, facing challenges like abuse, poverty, addiction, or mental health issues.

Develop treatment plans tailored to client needs.

Resource Coordination and referrals

Connect clients with the necessary services as required

Maintain a network of community resources and service providers

Counsellor

Provide individual or group counselling for stress, workplace conflicts, or personal issues

Assist employees in managing work-life balance and coping with occupational stressors

Crisis Responder

Support employees dealing with critical incidents such as workplace accidents or traumatic events.

Implement and coordinate post-incident debriefing sessions.

Mediator

Resolve conflicts between employees or between employees and supervisors.

Facilitate communication and understanding to improve workplace relationships Quality

Control

Organising clinical work environments , including infection control and risk management

Agent of Connectivity

Being an agent of connectivity between different health disciplines and between patients and health services
