Part - Time: 20 Hours Per Week 
1st Shift: 9am - 3:30pm
Monday - Friday: Flexible Days

Summary:

The Patient Educator Hematology collaborates with patients/families, patient’s school and/or homebound tutor, and other healthcare professionals involved with the patient’s care to assess learning needs.


Responsibilities:

1. Collaborates with patients/families, patient’s school and/or homebound tutor, and other healthcare professionals involved with the patient’s care to assess learning needs.

2. Plans and develops appropriate educational services for Hematology/Oncology patients in the inpatient unit and outpatient clinic setting, incorporating pertinent information from patient’s educational documents such as 504 plans and Individualized Education Plans.

3. Provides educational instruction in individual and/or group settings using age appropriate interventions to meet the specific needs of each patient.

4. Collaborates with Oncology Outreach Educator regarding identified learning difficulties or school concerns and school re-entry needs for patients served.

5. Maintain open and ongoing communication with home school/homebound tutor regarding patient educational progress.

6. Uses effective communication and collaboration to assess and promote patient’s educational goals

7. Plans and implements appropriate educational services for Hematology/Oncology patients in the inpatient and outpatient setting

8. Obtain and review pertinent educational documents such as 504 plans and IEPs and provide input into educational planning process

9. Maintains documentation of communication and educational services provided

10. Participates as an integral member of multi-disciplinary team

11. Demonstrates the knowledge and skills necessary to provide care for the physical, psych/ social, educational, and safety needs of the patients served regardless of age.

12. Other duties as required.


Other information:

Technical Expertise

1. Experience to make appropriate recommendations and interventions for patients' educational needs is required.

2. Experience working with all levels within an organization is required.

3. Experience in healthcare is preferred.

4. Proficiency in MS Office [Outlook, Excel, Word] or similar software is required.



Education and Experience

1. Education: Bachelor’s Degree in Education is required.

2. Certification: Valid State of Ohio teaching certificate [K-12] is required. BLS certification is required

3. Years of relevant experience: 3 to 5 years is required

4. Years of experience supervising: None.
