This position allows remote/work from home support for LTSS MMP HIV/TBI waiver participants in Christian, DeWitt, Logan, Macon, and Sangamon counties. We're excited to offer a $3,000 sign-on bonus, and the role includes 75% travel with mileage reimbursement based on the current IRS guideline rate.
Position Purpose: In this role, you will play a crucial part in helping our members access the services they need. You will coordinate and integrate medical and long-term care services, including orientation, care plan development, assessment, and care coordination.
Responsibilities:
- Conduct assessments with members, caregivers, or providers to gather information about their status, support system, and service needs for care plan development
- Monitor service delivery and follow up with members, caregivers, or providers through in-person visits and phone calls
- Authorize and coordinate referrals for services
- Ensure that provider services are provided without gaps and identify any deficiencies in care plans
- Assist in coordinating the development of informal or voluntary services to integrate into the member's care plan
- Collaborate with discharge planners, physicians, and other parties to ensure appropriate discharge plans, care plans, and coordination of acute care and long-term care services
- Support members in filing and resolving complaints and appeals
Education/Experience:
To qualify for this role, you need either:
- A Bachelor's degree or Registered Nurse licensure and 2+ years of care management experience
- An LPN and 4+ years of care management experience, or 6+ years of care management experience
Experience in home health, discharge planning, or long-term care is strongly preferred.
Brain Injury/HIV/AIDS:
Candidates must meet one of the following criteria:
- 1. A Registered Nurse (RN) licensed in Illinois and a bachelor's degree in nursing, social work, social sciences, or counseling, or four (4) years of case management experience
- 2. Certified or Licensed social worker with a Bachelor's degree in social work, social sciences, or counseling, or a Master's in social work
- 3. Unlicensed social worker with a minimum of a bachelor's degree in social work, social sciences, or counseling
