Job Responsibilities:

In this role, you will be responsible for coordinating patient care, performing assessments, and implementing care plans in accordance with physician orders and regulations. You will ensure the delivery of care that is compliant and meets the needs of patients and their families.

Qualifications:

• Graduate of an approved school of clinical education
• Current state license
• At least one year of experience as a licensed professional
• Basic technology skills, including the use of mobile devices

Preferred Qualifications:

• Review and approval from regional and executive service line clinical leadership to defer experience requirement
• Experience in Medicare home health or hospice

Requirements:

• Valid state driver license
• Automobile liability insurance as required by law
• Dependable transportation in good working condition
• Ability to safely drive in all weather conditions
• CPR certification for healthcare provider
