Home Health with Less Travel!

 

We are looking for Registered Nurses (RNs) to provide care to clients in various senior living communities, including assisted living and independent living facilities.

 

As an Enhabit RN, you will enjoy:

• Providing care within the assisted living facilities where our patients reside
• 1:1 patient care
• Following each patient from start to discharge

As an Enhabit RN, you will work collaboratively with facility wellness staff to ensure excellent communication and continuity of care for the residents and their loved ones. You will also consult with physicians and the office on patient care and serve as the point of contact for all disciplines involved in providing care. Your responsibilities will include accurate OASIS collection, maintaining up-to-date medication profiles, timely reporting of lab values to physicians, attending weekly case conferences and monthly case manager meetings.

Registered Nurses (RN) Case Managers must meet the following requirements:

1. Currently licensed as a Registered Nurse (RN) in the state of employment
2. Minimum of one year of clinical experience preferred
3. Knowledge and skill in current nursing practice
4. Valid state driver’s license and automobile liability insurance
