Your Role: As part of our team, you will work closely with our members and healthcare providers to ensure that their needs are met. This includes closing care gaps, removing barriers to care, conducting outreach programs, and providing face-to-face support. Your mission will be to enhance the overall member and provider experience, improve HEDIS measures, deliver education on coding, and support quality improvement interventions. You will also assist in scheduling appointments, arranging transportation, and connecting members with community-based resources when needed. Confidentiality and respect for privacy are paramount in this role.
Your Responsibilities:
- Act as a liaison and advocate for members, their families, and healthcare providers.
- Collaborate with providers to ensure compliance with HEDIS measures and offer guidance on coding.
- Analyze provider performance data to identify improvement opportunities.
- Work with Provider Relations to enhance overall performance in areas like Quality, Risk Adjustment, Operations, etc.
- Schedule doctor appointments for members and deliver face-to-face education on care gaps and barriers to care.
- Conduct telephone outreach and provide health coaching to support improvement initiatives.
- Arrange transportation and follow-up appointments for members, when necessary.
- Maintain detailed documentation of all member interactions.
- Collaborate with other departments, such as customer service, to resolve member issues.
- Refer members to case or disease management programs as needed.
- Complete special assignments and projects as assigned.
Your Qualifications: You should have a Bachelor's Degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or a related field. Alternatively, equivalent work experience in direct patient care, social work, quality improvement, or health coaching will be considered. Preferred qualifications include a license or certification as a Licensed Practical Nurse (LPN), Licensed Master Social Worker (LMSW), Certified Social Worker (C-SW), Licensed Social Worker (LSW), or Registered Nurse (RN).
