Supervisor of Care Management

Position Purpose:

As a Supervisor of Care Management, you will be responsible for implementing our Care Management programs in multiple areas including Prior Authorization, Concurrent Review, and Care Management. Your role will involve managing daily operational processes to ensure a seamless continuum of care for our members. We are looking for someone who embraces a patient advocacy approach, prioritizes continuity of care, and is committed to assisting members in accessing necessary services.

Your Responsibilities:
  • Supervise and support a team of care managers, including hiring, training, and performance evaluations.
  • Ensure appropriate triage of members at risk and supervise the assessment of members' needs.
  • Develop comprehensive care management plans based on assessment information.
  • Coordinate activities and resources to meet members' needs.
  • Identify and address deficits between member requirements and available benefits.
  • Collaborate with other departments to monitor programs and evaluate their effectiveness.
  • Maintain relationships with care management vendors to ensure compliance with policies and regulations.
  • Communicate with providers, members, and community resources to support the planning and implementation of care plans.
  • Conduct audits and contribute to corrective actions as needed.
  • Perform Care Management activities as required.
Qualifications:
  • CCM certification and a Baccalaureate degree preferred, or MSW with appropriate care management experience.
  • Minimum five years of clinical experience and at least two years of supervisory experience.
  • Current, valid, and unrestricted professional licensure required. Current, valid, and unrestricted RN licensure preferred.
