We are currently seeking a Per Diem Registered Nurse, Care Coordinator to join our team. In this role, you will work a minimum of 4 shifts per month on weekends. Your responsibilities will include:

  • Collaborating with various healthcare professionals to ensure the best possible care for each patient, taking into account their medical, psycho-social, financial, and practical needs.

  • Coordinating the care and services provided to our patient population from admission to discharge.
  • Assessing, planning, implementing, monitoring, and evaluating options and services to meet individual health needs, aiming for a safe and timely discharge to the next appropriate level of care.

Educational Requirements:

  • Preferred: Bachelor's degree in Nursing
  • Required: Associate's degree in Nursing and a current CA License

Licenses:

  • Preferred: Case Management certification and experience
  • Expected within 2 years of hire: Case Management certification
  • BLS certification required

Experience:

  • Minimum of 3 years in Acute Care Nursing

Physical Demands:

  • Standing, Walking
