Job Description

Position: Health Care Coordinator

Overview
As a member of our caring and inclusive team, you will work at a NORC site location, providing support and assistance to residents within the program. Your role will involve integrating health-focused information, education, assessments, advocacy, and linking residents to healthcare services. Your efforts will help residents maintain successful community living and self-care management.

Responsibilities
  • Collaborate with the NORC site staff to ensure residents are engaging in self-care management health strategies.
  • Reach out to seniors who may benefit from supportive services provided by the program.
  • Implement health promotion activities, such as screenings, assessments, referrals, and presentations.
  • Assess residents' needs and provide support through telephone calls and home visits.
  • Refer residents to appropriate health services based on their assessed needs.
  • Act as a liaison between healthcare systems during times of transition, providing health care advocacy.
  • Participate in quality assurance and education programs as requested.
  • Document all nursing interventions provided at the site.
  • Refer residents needing direct patient care to physicians or appropriate health care services.
  • Contribute to special projects and perform assigned duties.

Qualifications
  • Licenses and Certifications: Hold a valid license and current registration as a Registered Professional Nurse in New York State. CPR/BLS Certification is also required.
  • Education: Possess an Associate's Degree in nursing. A Bachelor's Degree in nursing is preferred.
  • Work Experience: Have a minimum of two years of experience as an RN in LHCSA or CHHA. Prior experience working with an aging population and conducting clinical assessments of their health needs is also required.
