Overview
We are looking for compassionate and skilled nurses to provide high-quality comprehensive care to patients/clients. In this role, you will coordinate and manage multidisciplinary health care services for a group of patients/clients, ensuring that their physical, social, and psychological needs are met. You will work under moderate supervision and uphold VNS Health's philosophy, policy, goals, objectives, and Standards of Nursing Practice.
Responsibilities- Assess the needs of patients/clients by observing, interviewing, and examining them, taking into account their physical, social, and psychological aspects. Develop a comprehensive plan of care based on your findings.
- Understand and implement reimbursement protocols, determining the billing source for patient/client care. Consult with management on complex reimbursement cases.
- Coordinate care for the multidisciplinary team by meeting the needs of patients/clients. This includes referring them to other disciplines or community agencies, ordering necessary supplies/services, and ensuring their delivery.
- Document all services provided in accordance with regulations, requirements, and VNS Health policies. Maintain clear written and verbal communication with team members and management to ensure optimal patient/client care.
- Orient selected staff through field observation and provide feedback. Also, provide guidance, training, and mentoring to students participating in selected programs.
- Deliver nursing care in accordance with VNS Health's policies, procedures, and Standards of Nursing Practice. This may involve activities such as standing, sitting, bending, and stretching.
- Transport required medical equipment and supplies to and from patient/client homes/care facilities using a designated carrying case.
- Travel to diverse environments (e.g., elevated buildings, care facilities, single/multiple family homes) to deliver direct care to the patient/client, utilizing approved transportation options.
- Follow established workflows and maintain productivity to meet VNS Health goals.
- Pursue personal professional growth through attendance at in-services, training, continued education, and reading select journals.
- Perform other related duties and participate in special projects as assigned.
- Establish, implement, and revise a care plan in collaboration with the multidisciplinary team and based on the patient's goals. Continuously assess, plan, coordinate, counsel, teach, and supervise care delivery to meet patient needs.
- Supervise the care provided by Licensed Practical Nurses and Paraprofessionals through regular home visits, ensuring compliance with regulatory and VNS Health requirements. Update the treatment plan as needed.
- Manage, plan, coordinate, and execute the patient's safe discharge from service. Ensure adherence to VNS Health and regulatory requirements in physician's orders, discharge summary, and communication with team members, patients, and family members.
- Communicate and consult with the patient's MD, addressing any problematic situations that affect care. Manage and coordinate all aspects of your assigned caseload.
- Assist patients with activities of daily living, including positioning, moving, transferring, and lifting them as needed and appropriate.
- Participate in special projects and perform other assigned duties.
- Licenses and Certifications:
- Must have a valid license and current registration to practice as a registered professional nurse in New York State.
- Valid driver's license or NYS Non-Driver photo ID card may be required as determined by operational/regional needs.
- Education:
- Must have a degree in Nursing from an approved program.
- Work Experience:
- Minimum of one year of experience in a medical/surgical environment required.
