Position: Home Health Care Aide (PRN/On-Call - 4-Hour Shift - Onsite)

Summary:
As a Home Health Care Aide, you will provide personal care, assist with food preparation, support patients with exercises, and perform other activities of daily living as directed by physicians.

Responsibilities:

  1. Assist and provide personal care, help with food preparation, and support patients with activities of daily living, ensuring hygiene and proper nutrition.
  2. Perform simple procedures under the supervision of healthcare professionals, including medication administration.
  3. Assist with educational activities and habilitation if specified in the patient's plan of care and approved by relevant authorities.
  4. Help patients with homemaking or chores as required and allowed by the patient's plan of care.
  5. Document all care provided to patients in their medical records.
  6. Demonstrate the necessary knowledge and skills to meet the physical, psychological, social, educational, and safety needs of patients of all ages.
  7. Perform other duties as assigned.

Requirements:

  • Good communication skills, including the ability to read, write, and report clinical information verbally.
  • Preferred experience in pediatrics or working with individuals with developmental delays.
  • Experience collaborating with different levels within an organization.
  • Experience in the healthcare field is preferred.
  • Proficiency in MS Office (Outlook, Excel, Word) or similar software.
  • High School Diploma or equivalent.
  • CPR and First Aid certification required. Home Health Aide certification is preferred, or completion of a Home Health Aide training program.
  • Preferred experience of 0-2 years.
  • No supervisory experience required.
