What is today the Isabella Geriatric Center was founded over 100 years ago out
of a young woman's compassion for those in need. Isabella Uhl dreamed of creating a home for aged women who had no homes or families. Before her untimely death at age 27 of an incurable illness she asked her parents to fulfill that dream for her and on May 15 1875 Isabella Home opened its doors to 25 aged indigent women.Since that time as the need for services became greater and more complex we embarked on a visionary program of expansion that also broadened Isabella's scope and its mission. In addition to adding skilled nursing beds we looked beyond traditional nursing home walls to develop programs that met the changing needs of older adults in a rapidly changing society.

Job Description

The Neighborhood NORC(NNORC) Nurse  provides healthcare assistance  and management for identified NNORC members with a focus on proactive disease prevention and successful  management of their chronic illness. The NNorc Nurse empowers members with skills commensurate with their own capacity to manage their own self-care, and provides necessary monitoring t, support, linkage, and coordination to enable them to be most effective in optimizing their health. The nurse identifies healthcare trends and issues within the
NNORC community and makes recommendations to address them.

DUTIES AND RESPONSIBILITIES:

  1. Provides regularly scheduled blood pressure screening sessions for NNORC residents. A brief health assessment is conducted during these sessions to all participants and blood pressure monitoring is provided overtime.

  2. Performs initial and periodic healthcare assessments of NNORC members identified by social worker to be at-risk. Determines their need for healthcare assistance or healthcare management.    

  3. Provides health education, medication review, provider referrals, linkage, advocacy, monitoring of blood pressure and chronic illness, as well as healthcare coordination to the extend needed, helping individuals manage their chronic conditions, respond to acute episodes, and access healthcare.

  4. Performs home visits, including safety assessments to prevent falls, in collaboration with social worker.  

  5. For clients in need of Healthcare Management, develops an assessment and care plan, arranges and coordinates services on the resident’s behalf. And follows up at least every two months.  

  6. Completes Health and Wellness assessments, Nursing Checklist, Progress Notes and Worker Log for units of servicedocumentation, as well as any other recordkeeping and reports.

  7. Helps clients access the healthcare system by providing assistance with scheduling
    appointments, coordination with pharmacies and physicians, and linkage to home
    care aides and other providers, as needed.

  8. Preforms other related duties

Qualifications

Requirements: 

  • Graduate of an accredited nursing program, bachelor’s
         degree preferred.

  •  
  • At least one year of homecare or community health
         nursing is strongly preferred. Experience performing Nurse Health
         Assessments and Developing Care Plans.

  •  
  • License/Registration: Current RN or NP in New York
         State
