Job Description
An exciting opportunity exists to join Iraq and Afghanistan Veterans of America as a Case Manager for the Rapid Response Referrals Program (RRRP). IAVA’s RRRP is our best in class one-on- one case management and referral service program. Since its launch in late 2012, our Case Managers have helped more than 5,900 veterans and family members get on track to meeting their goals through free and confidential support and connections to highest quality resources and benefits.
The Case Manager will work directly with Iraq and Afghanistan veterans and their families to understand their needs, provide information and referrals, crisis intervention services, and short-term case management. The Case Manger will be based in IAVA’s New York City headquarters and report to the RRRP Director.
KEY RESPONSIBILITIES
- Responsible for the day-to- day operations of IAVA case management through a combination of direct service and program referrals.
- Provide referral services to veterans in order to connect with internal and external service providers to address housing, employment, child care, financial assistance
and other needed services. - Identify strengths and needs of veterans and families and complete a basic needs assessment.
- Provide a benefits screening for veterans and identify eligibility for benefits.
Organize and prioritize daily services and responsibilities to ensure that veteran needs are met. - Provide crisis intervention to individuals and families facing emergencies around food, shelter, medical needs, spouse abuse, and any concrete need as presented by veterans or their families.
- Conduct follow-up to ensure veteran is in receipt of benefits and other services.
- Monitor compliance with contract requirements and program deliverables.
- Monitor and track cases in case management program.
- Utilize IAVA database (Salesforce) to maintain all records and documentation.
- Ensure that veteran records are complete, accurate and updated regularly.
- Prepare documents, reports, and statistics for all appropriate program-related activities.
- Participate in IAVA meetings, program reviews and site visits as required.
- Attend trainings and seminars to enhance skills related with job responsibilities.
- Other duties as assigned.
Qualifications
EDUCATION AND EXPERIENCE
- Bachelor’s degree in Social Work or a related field is required;
- MSW or the equivalent social service or mental health master’s degree required;
- Experience with and commitment to working with veterans and their families;
- Demonstrated ability to manage multiple projects with meticulous attention to detail;
- Excellent writing and verbal communications skills;
- Proactive and team-oriented;
- A dedication to quality – perfectionists welcome;
- A commitment to IAVA’s mission and core values.
