The dream of Hopewell Center began in the early 1950's when a group of parents sought services for their children and founded Peter Pan Preschool. The group incorporated in 1966 and officially adopted the name Hopewell Center in May 1983.

As a private, not-for-profit agency, Hopewell has experienced rapid growth in services, personnel, and persons served. While originally focusing exclusively on persons with disabilities, Hopewell has expanded its services to families with infants and toddlers that are 'at-risk' due to developmental or socio-economic factors.

Hopewell now has an array of facility and community-based opportunities, including family services, children's services, employment services, and residential services. Today, nearly 200 caring professionals provide person-centered services to over 450 individuals a year.

Job Description

The Rehabilitation Coordinator comprises the following summary of job duties, requirements and responsibilities.

Responsibilities

  • Coordinates and participates in Individual Service Plan (ISP) conferences, appropriately notifies all parties involved, and facilitates discussion and recommendation regarding consumers’ habilitation/rehabilitation goals for Day Program services.  Participates in  updating consumers’ ISP's and prepares all other required documentation accordingly.
  • Attends and participates in multi-disciplinary meetings, ISP meetings, and placement conferences; collaborating and sharing information to provide habilitation/rehabilitation to agency consumers.
  • Develops, revises measurable and effective training programs and oversees the implementation of consumers’ Individual Service Plan; collaborates with Supervisors in assessing consumer needs and developing functional objectives.
  • Reviews and submits monthly summaries of each consumer’s progress and current health and behavior status. 
  • Receives and addresses concerns from consumers, guardians and families on issues relating to Day Services.
  • Responds to emergency situations in the Day Services program and during community outings.
  • Monitors consumers’ development and progress by reviewing consumer files, training data, service notes and referral recommendations, communicating with consumer families and residential staff, observing consumers during habilitation activities, and working on a one-on-one basis with the consumer.
  • Acts as liaison between the Day Services program and ancillary service providers.
  • Prepares and submits a variety of reports and documents, as required, utilizing provided technology.
  • Assists in the coordination of program activities in the absence of the Day Services Director.
  • Assures health and safety of consumers on assigned caseload during Day Service hours.
  • Performs related duties as assigned, assuring the best interest of both the agency and the consumers' welfare.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, or related area, previous direct care experience in rehabilitation services.
  • Must obtain CPR and basic first aid certification.
  • Ability to effectively utilize basic technology platforms
  • Working knowledge and understanding of the Individual Program Plan (ISP) process, with proven ability to develop clear and concise goals and objectives for habilitation/rehabilitation of consumers.
  • Ability to supervise, direct, and provide technical assistance to Training Supervisors and Assistants and other personnel as assigned, including interviewing, hiring, orientation and training, evaluation and providing corrective instruction.
  • Ability to effectively communicate and collaborate with professional and non-professional personnel, consumers and their families and officials from other agencies.
  • Ability to understand and apply a variety of occupational manuals and professional journals to assigned area of responsibility.
  • Ability to work independently and to organize a wide variety of ongoing projects under minimal supervision and guidance.
  • Ability to follow all personnel policies and rules of the organization. 
  • Ability to maintain confidentiality.
