Job Description
As a Client Care Manager, you will manage your own portfolio of clients, ensuring they receive the highest standards of care and support.
You will work as part of a supportive team alongside two other Client Care Managers and the Head of Client Services. You will also be supported by a Medication Management Administrator.
Your client portfolio will be based within a defined geographical area, helping to minimise travel time between client visits.
You will have responsibility for overseeing all aspects of your clients’ care, ensuring that care is safe, effective, compliant and centred around their individual needs.
Key Responsibilities
- Manage your own portfolio of clients and maintain strong, professional relationships with clients and their families.
- Monitor the quality and delivery of care, ensuring it remains person-centred and meets the individual needs of each client.
- Ensure care is delivered in line with relevant legislation, CQC requirements and company policies and procedures.
- Review and update care plans when clients’ needs change.
- Support clients following hospital admission and discharge, particularly where their care needs have changed.
- Liaise with multidisciplinary teams, discharge planners, ward staff and other healthcare professionals to support safe and effective hospital discharge.
- Work closely with GPs, social workers, pharmacies, district nurses and other community services to ensure clients receive appropriate and coordinated support.
- Identify changes in clients’ needs and make appropriate referrals to community services and other sources of support.
- Work closely with the Business Development Manager, who will complete the initial client assessment, risk assessments, care plans and medication requirements before handing the client over to you.
- Provide professional advice, guidance and reassurance to clients and their families.
- Ensure accurate and timely records are maintained.
- Support the team in maintaining the highest standards of care, compliance and customer service.
Qualifications
About You
We are looking for someone who is caring, organised and confident, with strong experience in care management and a genuine passion for supporting older people.
Essential Requirements
- At least 2 years’ experience in a care management role.
- Experience writing and reviewing care plans.
- Good knowledge of CQC regulations and the Key Lines of Enquiry (KLOEs).
- Experience using people management or care management systems.
- Excellent communication and interpersonal skills.
- Strong organisational, time management and prioritisation skills.
- A professional and positive approach.
- Ability to work independently while also contributing effectively to a wider team.
- Flexibility to meet the needs of the business.
- Full UK driving licence and access to your own vehicle to visit clients.
- Local to New Malden or within a reasonable travelling distance. This is an office based role.
