Job Description

Objective:

The Care Coordinator is expected to perform a variety of duties including; Client Review and Client QA’s and CAREGiver support calls and Introductions.

Main Duties:
•    New Client CAREGiver Introductions
•    Completing service reviews as per the correct schedule in order for us to remain CQC compliant. Triage any urgent reviews based on the need, update PP
•    Undertaking QA visits as per the correct schedule for us to remain CQC compliant, update PP
•    Updating caregivers after service review and QA 
•    Updating and maintaining eMARs.
•    Updating the client and caregiver logs on People Planner 
•    To ensure successful operation of quality control systems 
•    Implementation of complaints procedure
•    Willing participation in ‘on call‘ duties 
•    Maintain working relationships and effective communication both within and across teams and services, and externally with partners.
•    Salary from £21,000 to £ 24,000 per year.

Qualifications

Experience of service provision in the care profession. Minimum standard of ‘working towards’ level 3 QCF diploma in care.
•    Excellent communication skills, both verbal and written
•    Commercial business awareness
•    Good planning and organisational skills
•    Skills in assessment and care planning 

Essentials:
•    Must have full driving licence 
•    To able to develop and maintain the quality control system 
•    To have a strong knowledge of the requirements of the Care Standards regulations 
•    To be able to create and maintain administrative systems 
•    To be able to establish and maintain effective working relationships
