Job Description

The role will support the weekend team by providing office cover on a Saturday and Sunday, the role will help field calls from CAREGivers and Clients and direct and resolve as appropriate.

The role is in entry level step into Care Management and is ideal for anybody looking to make the next step in their career in care.

Key Tasks

Handling calls from CAREGivers, Clients and partners

Scheduling to ensure any sickness calls are covered

Monitoring of Care Activities to ensure outcomes are met for clients

Updating Care Plans and Risk Assessments

Providing advice and liaising with Health and Social Professionals

Qualifications

NVQ/QCF Level 2 in Health and Social Care or Equivalent

IT Literate

Good knowedge of Microsoft Office Products

Committed Can do attitude
