Role Overview

As a Patient Navigator, you will play a vital role in coordinating access to health-related services for our clients. You will guide them through the healthcare system, ensuring they receive the care and resources they need. Your work will directly impact the well-being of our patients and strengthen our community ties.

What You Will Do

  • Identify potential clients, complete initial needs assessments, and maintain follow-up to ensure appropriate guidance to health services.
  • Provide informal counseling, education, and social support to clients.
  • Assist clients with utilizing available resources, including scheduling medical appointments and accompanying them to appointments.
  • Help clients complete medical program applications.
  • Develop and maintain strong working relationships with external and internal contacts.
  • Maintain accurate client records by documenting all contacts, services provided, and outcomes.

What You Will Need

Required Qualifications

  • Ability to identify potential clients and complete needs assessments.
  • Skills in providing informal counseling, education, and social support.
  • Experience in assisting clients with resource utilization and application completion.
  • Proficiency in maintaining accurate client records and documentation.

Preferred Qualifications

  • Experience working in a healthcare or community support role.
  • Familiarity with local health resources and community agencies.
