As a Case Manager, you are a critical member of the patient-centered care team, responsible for identifying, assessing, and coordinating care to meet the health needs of individuals and families. What You Will Do

• Collaborate with the care team to identify and assess patient needs.
• Develop and implement individualized care plans to promote patient safety and quality outcomes.
• Coordinate effective discharge planning to ensure a safe transition to the next level of care.
• Advocate for patients and families, addressing social determinants of health and connecting them with community resources.
• Evaluate care plans and outcomes, making adjustments to optimize health along the care continuum.

What You Will Need

Required Qualifications

• Bachelor's degree in nursing or a Master's degree in Social Work.
• Minimum 3 years of clinical experience.
• Excellent verbal communication, written documentation, and interpersonal skills.
• Strong problem-solving, analytical, and decision-making abilities.
• Proficiency in computer skills and knowledge.
• Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.

Preferred Qualifications

• Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC).
• Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
