We focus on meeting individuals where they are, providing care coordination, home health services, and support across diverse populations. Our team is dedicated to making a meaningful impact by connecting patients with the resources they need to thrive in their communities.

Role Overview

As an RN in Home Health Care, you will independently provide direct patient care in the home, utilizing your nursing expertise to assess and address patient needs. You will coordinate with allied health professionals to ensure a comprehensive and integrated care delivery system, supporting patients and their families in achieving optimal health outcomes.

• Document patient care electronically using computer systems.

What You Will Need

Required Qualifications

• Graduation from an accredited Registered Nurse program.
• Current licensure as a Registered Nurse in the State of Michigan.
• Ability to effectively communicate with patients, families, and healthcare personnel.
• Knowledge of computer use for electronic documentation.
• Current driver's license, satisfactory driving record, and reliable transportation.
• Ability to lift, carry, transport, and move clients as needed.

Preferred Qualifications

• Minimum of nine months to one year of clinical nursing experience.
