Areas Covered: Primarily Oakland County - Troy, Madison Heights, Clawson, Oak Park, Ferndale, Hazel Park, Huntington Woods, Birmingham, Pleasant Ridge and Royal Oak areas.

We focus on meeting individuals where they are, providing care coordination, and connecting them with resources such as physical therapy, occupational therapy, and social work. Role Overview

As an RN in Home Health Care, you will independently provide direct patient care in the home, utilizing your nursing expertise to assess and address patient needs. You will coordinate with allied health professionals to ensure a comprehensive care plan, supporting patients and their families in achieving optimal health outcomes. Your role is critical in delivering personalized, high-quality care that improves quality of life.

What You Will Do

• Provide direct patient care in the home, applying nursing principles to assess and meet patient needs.

• Document patient care electronically, maintaining accurate and up-to-date records.

What You Will Need

Required Qualifications

• Graduation from an accredited Registered Nurse program.
• Current licensure as a Registered Nurse in the State of Michigan.
• Ability to effectively communicate with patients, families, and healthcare personnel.
• Knowledge of computer use for electronic documentation.
• Strong interpersonal and customer service skills.
• Valid driver's license, satisfactory driving record, and reliable transportation.
• Ability to lift, carry, transport, and move patients as needed.

Preferred Qualifications

• Minimum of nine months to one year of clinical nursing experience.
