What You Will Do
- Identify, assess, and plan care strategies for patients, ensuring their health care needs are met.
- Facilitate care coordination and advocate for patients and their families.
- Provide discharge planning services to support safe transitions to the next level of care.
- Manage and address the needs of patients with critical diagnoses or events.

Qualifications
What You Will Need
Required Qualifications
- Bachelor's degree in nursing or Master's degree in Social Work.
- Minimum 3 years of clinical experience.
- Excellent verbal and written communication skills.
- Strong customer service and interpersonal skills.
- Proficiency in problem-solving, analytics, and decision-making.
- Computer literacy and knowledge of healthcare systems.
- Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid Michigan license.
Preferred Qualifications
- Experience in discharge planning, home health care, or managed care.
- Certification in Case Management (CCM) or Accredited Case Manager (ACM).
