Role Overview

As a Patient Care Coordinator, you¿ll play a critical role in facilitating patient admissions and transfers based on acuity, bed availability, and required services. You¿ll collaborate with nursing staff and multidisciplinary teams to ensure patients receive the appropriate level of care, acting as a liaison between physicians and specialty consultants to achieve optimal outcomes.

What You Will Do

  • Collaborate with nursing staff and multidisciplinary teams to place patients in the appropriate level of care.
  • Coordinate as a liaison between physician requestors and specialty consultants.
  • Initiate the care management process, including pre-registration and insurance verification.
  • Review medical reports from referring hospitals or physician offices and relay information to the nursing unit.
  • Maintain documentation of communications for legal, medical, statistical, and process improvement purposes.
  • Coordinate education and development of internal staff and external departments on intake review and approval processes.

Qualifications

What You Will Need

Required Qualifications

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
  • Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date.
  • Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
  • Excellent computer skills.
  • Familiarity with surgical terminology.
  • Ability to multitask.

Preferred Qualifications

  • Experience in patient admissions or care coordination.
  • Knowledge of managed care and insurance verification processes.
