As a Case Manager, you will be a vital member of the patient-centered care team, responsible for identifying, assessing, planning, and coordinating care to meet the health needs of individuals and families. What You Will Do

  • Collaborate with the healthcare team to identify and assess patient needs.
  • Develop and implement care plans that promote patient safety and quality of care.
  • Coordinate services and resources to support patients' health outcomes.
  • Provide effective discharge planning to ensure a safe transition to the next level of care.
  • Advocate for patients and families, addressing their needs and concerns.
  • Evaluate care plans and outcomes to optimize health along the care continuum.

Qualifications

What You Will Need

Required Qualifications

  • Bachelor's degree in nursing or a Master's degree in Social Work.
  • Minimum 3 years of clinical experience.
  • Excellent verbal communication and written documentation skills.
  • Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.

Preferred Qualifications

  • Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC).
  • Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
