Job Description

Physical Therapist - Home Health Care

Location: Oakland County
Schedule: 40 hours per week, day shift
On-Call Requirements: Minimum of one weekend per month, plus one summer holiday and one winter holiday
Compensation & Benefits: $10,000 sign-on bonus; eligible for full-time benefits

**Job Description:**

Ready to make a real difference in people's lives? As a Physical Therapist in our Home Health Care team, you'll bring movement, strength, and hope back to our rehabilitative clients right in their own homes! Working collaboratively with our therapy supervisor, you'll design and deliver personalized physical therapy treatment programs that get results—whether that's easing pain, restoring mobility, or helping clients reclaim their independence.

**Key Responsibilities:**

  • Implement medically prescribed physical therapy treatment programs tailored to each client's unique needs
  • Get creative with diverse therapeutic techniques—from heat and aquatic therapy to targeted exercises and functional training
  • Work one-on-one with clients to relieve pain and restore physical function in a supportive, encouraging environment
  • Track progress and celebrate wins with your clients as they reach their rehabilitation goals
  • Collaborate with the therapy supervisor and broader healthcare team to ensure coordinated, high-quality care
  • Maintain detailed documentation and adapt treatment plans based on client progress and feedback
  • Bring your expertise, compassion, and positive energy to every home visit

Qualifications

EDUCATION:

A specific level of technical knowledge and the applications of various treatment interventions in order to relieve pain and/or restore a physiological function.

  • This level of knowledge is generally obtained through the completion of an accredited bachelor's level program in Physical Therapy.

LICENSE/CERTIFICATIONS:

  • Current State of Michigan Licensure in order to practice as a registered physical therapist.

QUALIFICATIONS:

  • The ability to lift and position clients and to perform treatment modalities.
  • A high level of interpersonal skill is required to effectively interface with clients and physicians.
  • Must possess a current State of Michigan driver's license and satisfactory driving record.
  • Must have reliable transportation and a telephone.
