Job Description
Physical Therapist - Home Health Care – Northern Oakland County - 20 Hours/Week
Eligible for a 5,000 sign on bonus
As an Physical Therapist in our Home Health Care Division, you will be required to conduct Therapy in our patients homes.
**General Summary**

As a Physical Therapist in Home Health Care, you'll partner with our therapy supervisor to deliver personalized, medically prescribed physical therapy programs that get results! Whether you're using heat therapy, aquatic exercises, or innovative rehabilitation techniques, you'll help our clients overcome pain and reclaim their physical independence—one session at a time. This is hands-on, meaningful work where you'll see the direct impact of your expertise every single day.
**Key Responsibilities:**
- Design and implement customized physical therapy treatment programs tailored to each client's unique needs and goals
- Deliver engaging therapy sessions using a variety of techniques (heat therapy, water-based exercises, therapeutic exercises, and more) to relieve pain and restore physical function
- Work collaboratively with the therapy supervisor and interdisciplinary care team to ensure seamless, coordinated client care
- Track client progress and celebrate wins—adjusting treatment plans to maximize rehabilitation outcomes
- Provide clients with practical guidance and encouragement to support their recovery journey at home
- Maintain detailed, accurate documentation of all therapy sessions and client progress
- Build strong, supportive relationships with clients and their families to foster trust and engagement in the rehabilitation process
Qualifications
EDUCATION AND EXPERIENCE:
- Accredited Bachelor's Degree in Physical Therapy required to relieve pain and/or restore a physiological function.
- Requires current Physical Therapist licensure in the State of Michigan.
- Must be able to lift and position clients and perform treatment modalities.
- High level of interpersonal skill is required to effectively interface with clients and physicians.
- Must possess a current State of Michigan driver's license and a satisfactory driving record.
- Must have reliable transportation and a telephone
